Interesting data from NHS Improvement has revealed that 24 severe medical errors have been recorded throughout Greater Lincolnshire Hospitals since 2017.
Such errors are logged as ‘never events’, referring to serious incidents which are completely preventable and have to potential to cause anything from significant harm or death to patients.
What types of errors were recorded?
Here’s a breakdown of the types of mistakes that were made which were classed by the report as never events:

- Wrong site surgery – Most common incident reported, with ten cases reported across both trusts in the three-year timescale.
- Swab left inside patient.
- Retained foreign objects.
- incorrect size implant.
- Misplaced naso-gastric tube.
The Grantham Journal also highlighted that the option for commissioners to impose fines for errors was removed in January 2018, and NHS Improvement stating that such practices “reinforced blame culture.”
