During investigations, Ms F attended the hospitals emergency department on 17 July 2020 and was discharged negligently without consideration of appendicitis. Ms F. presented again at the emergency department on 20 July 2020, and again was failed to be diagnosed and treated for appendicitis.

Due to the delays in diagnosing and performing an appendectomy, this led to the rupturing of the appendix which caused sepsis. Had the hospital correctly identified and treated Ms F, it is likely that she would have survived.
Our thoughts and condolences are with the loved ones of Ms F following these tragic and avoidable circumstances.
